Photos help a dentist give general information, but they cannot show decay between teeth, gum pockets, bone levels or the bite, all of which change what treatment is needed. A firm figure needs an examination and, where indicated, X-rays.
It depends on your policy. Many plans cover basic dental care with limits, and cosmetic treatment is commonly excluded. Check your table of benefits and ask your insurer whether pre-approval is needed before treatment starts.
