Published with the patient’s consent. In the photo, the top half shows the teeth before treatment and the bottom half after. Every mouth is different: your own plan follows an examination.

Before: the situation

The upper front teeth are square with flat edges and noticeably lighter than the lower teeth. The lower teeth are yellow, with brown stain lines between the lower incisors and chalky, worn patches on the lower side teeth. A dark grey area is visible on an upper back tooth on one side, and the upper front teeth cover a large part of the lower front teeth when the patient bites.

What was wrong, and why it matters

When upper and lower teeth differ in colour and condition, the smile looks unbalanced, and restoring only the more visible teeth leaves the contrast in place. Brown lines between teeth and chalky worn patches suggest that the enamel surface is breaking down, and these areas pick up stain and are prone to further wear and decay.

Wear across both arches also affects the bite. As teeth flatten, the jaw can close further than it should, the front teeth may meet at an awkward angle, and the chewing muscles and jaw joints adapt to the change. A dark area on a back tooth can mean an old restoration or decay, and it needs to be checked as part of the plan.

The solution

The after photo shows the upper and lower teeth in the smile in one uniform, bright shade, with smooth surfaces and rounded edges. The stain lines, chalky patches and dark area seen before are no longer visible, and the upper and lower teeth now match.

In a case covering both arches, Dr. Marina plans the bite before the aesthetics: the jaw joints, chewing muscles and tooth contacts are assessed, and the new tooth shapes are designed so both arches work together. She attended Prof. Marcelo Calamita’s course “Esthetics & Integrating Occlusal Principles into Smile Design” in Dubai in February 2025, which deals with exactly this link between occlusion and the smile.

How a case like this is planned and carried out

  1. Full examination with photographs, X-rays and a digital scan of both arches

  2. Functional assessment of the bite, TMJ and chewing muscles, with axiography and condylography where indicated

  3. Digital smile design and a mock-up for the upper and lower teeth

  4. Treatment in planned stages, arch by arch

  5. Restorations made with the ceramist in the dental lab in the same building

  6. Bite reviewed and refined at follow-up visits

After: the result

Upper and lower teeth now share one bright, uniform shade, with smooth, rounded edges and no visible stains or worn patches. The lower teeth match the upper teeth, so the smile reads as one set.

Questions about this kind of case

When does a smile need work on both upper and lower teeth rather than only the front six?

When the lower teeth are worn, stained or decayed, or when the bite has changed through wear, treating only the upper front teeth leaves a visible contrast and can strain the new restorations. Planning both arches lets shade, tooth length and contacts be set together. Dr. Marina Antoniuk at Creative Arts in Dubai decides the scope after examination and a functional assessment of the bite.

How is the bite checked after new restorations on both arches?

After fitting, the dentist checks the contacts as the teeth close and as the jaw moves forward and sideways, and adjusts any points that touch too early or too hard. The patient returns for review once they have used the new teeth for a while, and a night guard may be discussed if there are signs of grinding. Dr. Marina Antoniuk follows this sequence at Creative Arts in Dubai.
