What are Invisalign attachments?
Invisalign attachments are small, tooth-coloured shapes made of composite filling material, bonded to the front of selected teeth. They give the aligner a surface to grip and push against, which helps it rotate teeth, move roots and pull teeth down or up. They are visible up close but blend with the teeth, and they are polished off when treatment ends. What do Invisalign attachments actually do?
An aligner is a smooth plastic shell. On its own it can push a tooth in a straight line fairly well, but teeth are rounded, and some movements need more grip than a smooth surface allows. Attachments solve this by adding small, precisely shaped bumps to the tooth surface. The aligner is moulded to fit around them, so when the aligner pushes, it has an edge to push on.
Attachments are typically used for:
- Rotations, especially of rounded teeth such as canines and premolars, which tend to slip inside a smooth aligner.
- Extrusion and intrusion, pulling a tooth down or pushing it up, which a smooth tray cannot grip well.
- Root movement, so that a tooth moves upright rather than just tipping its crown.
- Retention of the aligner, helping the tray stay seated when other movements are pulling against it.
- Elastics, where small hooks or buttons may be added to attach rubber bands for bite correction.
Each attachment's shape, size and position is chosen in the digital plan for the specific movement that tooth needs, which is why they look different from tooth to tooth.

Are Invisalign attachments noticeable?
They are designed to be discreet rather than invisible. The composite is chosen to match your tooth shade, but each attachment has edges and a slightly different surface texture, so it can catch the light at close range. Most people around you will not notice them, especially with the aligners in, because the clear plastic covers them and evens out the surface. You will feel them with your lips and tongue for the first few days.
Attachments can pick up surface stain over the months, particularly from coffee, tea, karak or smoking, and this is more noticeable on attachments than on enamel. How are Invisalign attachments put on?

Clean and prepare
The selected teeth are cleaned and dried. A mild gel briefly roughens a small area of enamel so the material can bond.
Load the template
A soft template aligner with small hollows in the planned positions is filled with tooth-coloured composite.
Seat and cure
The template is pressed onto the teeth and a curing light hardens the material in each hollow.
Remove and tidy
The template comes off, leaving the attachments in place. Any excess material is cleaned away and the bite is checked.
Fit the first aligner
Your first aligner is fitted over the attachments, and you are shown how to put it in and take it out.
Does removing Invisalign attachments damage teeth?
At the end of treatment the attachments are removed with a fine polishing instrument that takes the resin back to the tooth surface, followed by polishing the enamel. Because the material was bonded to the surface rather than placed in a drilled cavity, the aim is to remove the attachment while leaving the enamel underneath intact. A careful clinician works slowly, under good light and magnification where available, to avoid taking off enamel along with the resin.
After removal, teeth can feel unusually smooth or slightly rough for a day or two until you get used to them. Faint marks where an attachment sat are usually polished out at the same visit. If the teeth around an attachment were not cleaned well during treatment, white marks can show up after removal; these come from plaque, not from the attachment itself, and are one of the reasons good cleaning matters throughout. Your retainer is usually made or fitted around the time attachments come off.
An attachment has come off. What now?
Keep wearing your aligners and check that the current one still seats fully. Contact the clinic during opening hours, 10:00 to 20:00 daily, so the attachment can be replaced at your next visit or sooner if the aligner no longer fits well. Do not try to glue anything to your teeth yourself.
Can you have Invisalign without attachments?
Sometimes. Very simple cases, such as small amounts of spacing or minor crowding of front teeth that only need tipping, may need few or no attachments. But for most cases, attachments are what make the plan work as designed. Asking for treatment without them usually means accepting that rotations, root movement or bite changes will be less predictable, which can lead to more aligners, more refinements or a compromise result.
A better question is whether you could have fewer, or have them placed later, or have them on less visible teeth. Your orthodontist can sometimes adjust the plan, and can explain which movements depend on which attachments. Placing attachments does not usually hurt. The first days afterwards can feel strange: lips and cheeks notice the new bumps, and aligners fit more tightly and can be harder to remove, especially at the back. That tightness is a sign the aligner is gripping as intended. Taking aligners out from the back teeth first, using your fingertips rather than nails, makes removal easier.
