Custom or pharmacy

Is a pharmacy night guard enough, or is a custom one worth the cost?

A pharmacy boil-and-bite guard costs much less and can be a reasonable stopgap for a short time, for example while you wait for an appointment or travel. It is softened in hot water and bitten into, so it is bulkier than a custom guard and does not match the way your teeth meet. Some people find they chew on a thick, soft guard, and heavy grinders can wear through one quickly.

A custom guard is made from a scan or impression of your teeth, so it is thinner and closer-fitting, and its biting surface is adjusted so your opposite teeth touch it evenly. That is what makes it comfortable enough to wear every night, and a guard left in its case protects nothing. How often does a night guard need replacing?

It depends on how hard you grind. Some guards last years; others show deep wear marks much sooner. Bring your guard to check-ups: the fit is checked, and the wear pattern tells your dentist how heavily you are grinding. Replace or remake it if it cracks, wears through, becomes loose, or stops fitting after new crowns, fillings or orthodontic treatment.

Care makes a difference too. Rinse and gently brush the guard each morning, keep it in its case, and never use hot water, which can distort it. What a night guard does, and what it does not

A 2007 Cochrane review found that splints may protect teeth from wear, but there was not enough evidence that they reduce the grinding itself. A night guard is protection, not a cure. If grinding comes with jaw pain, headaches around the temples or clicking with pain, the NHS lists these among the symptoms of temporomandibular disorder, and a jaw assessment is worth having. Other approaches some clinics offer for grinding are outside the scope of this page; ask at your assessment.

Protecting restorations

Is a night guard worth it if you have veneers, crowns or implants?

For people who grind, usually yes. Grinding is one of the most common reasons ceramic chips and restorations wear early, and repairing or replacing a single veneer or crown costs far more than a guard. That is why a night guard is often part of the plan after veneers, a Hollywood smile or a full-mouth case, and why a hard, precisely adjusted guard is usually preferred when restorations are involved.
