Wisdom Teeth and Braces: Remove Them Before, During or After Treatment?

Most people can have braces or clear aligners with their wisdom teeth in place. They are removed when they cause problems or block a planned movement, not as a routine step before straightening.

Do wisdom teeth need to be removed before braces?

Usually not. Most people can start braces or Invisalign with their wisdom teeth in place. Removal is advised when a wisdom tooth is causing problems, blocks a planned tooth movement, or is likely to become stuck against the second molar. Research has not shown that wisdom teeth push straightened teeth crooked; wearing retainers matters far more. Why do people think wisdom teeth crowd the front teeth?

The idea is understandable. Wisdom teeth usually appear between the late teens and mid-twenties, and that is also when many people notice their lower front teeth becoming slightly more crowded, sometimes years after braces came off. It seems logical that the new teeth at the back are pushing the others forward.

The research does not support that explanation. Lower front teeth tend to become a little more crowded with age in many people, including those who never had wisdom teeth, because of continuing jaw growth, the forces of chewing and the natural tendency of teeth to drift forward over a lifetime. A 2014 systematic review in The Scientific World Journal concluded that definitive conclusions on the role of wisdom teeth in crowding cannot be drawn, that most studies show no causal link, and that removing wisdom teeth to prevent crowding or relapse after orthodontic treatment is not justified. A 2023 systematic review in Dentistry Journal likewise found no proven connection between lower wisdom teeth and the return of lower front crowding after treatment.

In other words, crowding that comes back after braces is mainly a retention question, not a wisdom tooth question.

When might an orthodontist recommend removing wisdom teeth?

Removal is decided tooth by tooth, for a specific reason. These are the situations in which it is usually discussed.

What does the orthodontic assessment check about wisdom teeth?

Wisdom teeth are part of every orthodontic assessment, even though they are rarely the focus. On the panoramic X-ray the orthodontist looks at:

  • Whether they are present and how far they have developed; some people have fewer than four.
  • Their angle and space: whether there is room behind the second molars or whether a lower wisdom tooth is already tilting against the tooth in front.
  • Their effect on the plan: if the back teeth need to move backwards, the wisdom teeth may decide how much movement is possible.
  • Any disease, such as decay in the wisdom tooth or the second molar, or changes in the bone around them, which would need attention regardless of orthodontics.

The answer for most people is that the wisdom teeth can be left alone and monitored. A 2020 Cochrane review of removing versus keeping symptom-free impacted wisdom teeth found that the included studies showed no clinically significant effect of removal on crowding, and advised regular monitoring of teeth that are kept.

Do wisdom teeth matter more with Invisalign than with braces?

The biology is the same, but there are a few practical differences with clear aligners. Aligners are made from a digital scan of the teeth that are present at the time. If a wisdom tooth has already erupted, it may simply be included in the aligner; if it is still under the gum, the aligner usually stops at the second molar.

A wisdom tooth that comes through partway during treatment can stop the back of an aligner fitting properly, or make the gum sore under the edge. This is usually managed by trimming the aligner or by a new scan. When the plan includes moving the molars backwards to create space, which aligners can do in some cases, the orthodontist checks on the X-rays whether the wisdom teeth leave room for that movement before treatment starts.

Will removing wisdom teeth after braces stop teeth moving back?

No. Teeth tend to drift after braces or aligners whether or not wisdom teeth are present, and removing them does not replace the job a retainer does. The fibres around the teeth and the soft tissues keep exerting gentle pressure long after treatment ends, which is why retention is long-term.

At the end of treatment you will usually be given a bonded wire behind the front teeth, a removable retainer, or both. Wearing them as instructed is what protects the result. Our retainers page explains the types, and the guide to how long to wear a retainer covers the schedule.

If your teeth have moved since treatment, the cause is worth investigating rather than assuming it is the wisdom teeth: a broken bonded wire, a retainer that has not been worn, or changes in the gums are all more common explanations.

Do not have healthy wisdom teeth removed just to protect your braces result

Removing wisdom teeth is surgery, with recovery time and its own small risks. If a wisdom tooth is healthy and does not affect your orthodontic plan, the evidence does not support removing it to prevent crowding. If it causes problems in the future, it can be removed then, for that reason.

What happens to wisdom teeth during teenage orthodontic treatment?

In teenagers, wisdom teeth are usually still developing when braces or aligners start. They show up on the panoramic X-ray taken at the orthodontic assessment, often as tooth buds without roots. At that age it is rarely possible to tell whether they will erupt normally, so the usual approach is to note them and check them again on later X-rays.

Towards the end of treatment, or a few years afterwards, the orthodontist or general dentist can usually see whether there is enough space. If it looks as though a lower wisdom tooth will become impacted against the second molar, removal can be planned at that stage on its own merits. Do wisdom teeth ruin braces results?

The evidence does not show that wisdom teeth push straightened teeth out of line. Lower front teeth can become slightly more crowded with age in many people, with or without wisdom teeth. The most important protection for a braces or aligner result is wearing your retainers as instructed, not removing wisdom teeth.

Should wisdom teeth be removed before Invisalign?

Usually not. Invisalign can be carried out with wisdom teeth present. Removal before treatment is considered when a wisdom tooth is causing problems, or when the plan involves moving the back teeth backwards and the wisdom tooth leaves no room. Your orthodontist checks this on the X-rays before the aligners are designed.

Can I get braces while my wisdom teeth are coming in?

Yes. Braces and aligners do not need to wait until wisdom teeth have erupted. The orthodontist notes their position on the X-rays and monitors them. If a wisdom tooth starts causing gum soreness or infection during treatment, it can be dealt with without stopping the orthodontic treatment.

Can wisdom teeth be removed during braces treatment?

Yes. If a wisdom tooth needs to come out while braces or aligners are on, the extraction can usually be done without removing the appliance. The orthodontist and the surgeon coordinate the timing, and you may be asked to skip an adjustment or wear aligners slightly differently while the area heals.

Will removing my wisdom teeth after braces keep my teeth straight?

No. Teeth tend to move after orthodontic treatment whether or not wisdom teeth are present. Retainers are what keep teeth straight. If your teeth have started to move, check whether your retainer still fits and whether a bonded wire has come loose, and ask your orthodontist to assess the cause.

Why did my orthodontist say my wisdom teeth can stay?

Because they are not affecting the planned tooth movement and show no signs of disease. Current evidence does not support removing healthy wisdom teeth to prevent crowding, and removal is surgery with its own recovery. They will continue to be checked on X-rays, and removed later if a reason appears.

Can a wisdom tooth replace a missing molar with braces?

In some cases, yes. If a first or second molar has been lost or is beyond repair, an orthodontist may be able to move the teeth behind it, including a wisdom tooth, forward into the space. Whether that is realistic depends on the wisdom tooth's position, roots and the rest of the bite.
