Sharp pain on releasing the bite

A short, sharp jolt when you let go after biting something firm is a classic sign of a cracked tooth: the pieces flex apart under load and snap back. The American Association of Endodontists describes this erratic chewing pain.

Dull ache when you press down

A deep, bruised feeling when you bite or tap the tooth, which can linger, usually comes from the ligament around the root. Inflammation or infection at the root tip and heavy clenching are the usual reasons.

If the pain began within days of a filling or crown, a slightly high contact is the first thing to check. It is quick to correct, and the soreness usually settles within days afterwards.

Diagnosis

How does the dentist find which tooth hurts on biting, and why?

Bite pain is often hard to place: many people point to the wrong tooth, or to the upper jaw when the problem is in the lower. The examination works through simple tests, one tooth at a time:

  • Bite test. You bite on a small plastic tool placed on one cusp at a time. A crack usually hurts on one cusp, and often on release.
  • Tapping. Gentle tapping on each tooth shows whether the ligament around a root is inflamed.
  • Cold test. How the tooth reacts to cold, and how long the feeling lasts, shows whether the nerve is healthy, inflamed or no longer responding.
  • Bite check. Thin marking paper shows whether a filling or crown touches first or too heavily.
  • X-ray. Shows decay under fillings and changes around the root tip. A fine crack often does not show, which is why the clinical tests matter.
  • Magnification. Removing an old filling and looking under the dental microscope can reveal a crack line that cannot be seen otherwise.

When to get help quickly

Book promptly if a tooth hurts on biting and also throbs on its own, wakes you at night, or the gum next to it swells; delaying can allow an infection to spread. Go to a hospital emergency department straight away for facial swelling with fever, difficulty breathing or swallowing, swelling spreading towards the eye or neck, uncontrolled bleeding, or a head or face injury. Chew on the other side and avoid hard, crunchy or very chewy food, such as nuts, ice, hard bread crusts and toffee, which flex a cracked tooth and load an inflamed one. If you clench during the day, notice it and let your teeth rest apart. Over-the-counter pain relief taken as directed on the label usually controls the discomfort; ask a pharmacist which suits you if you take other medicines.

Is it normal for a tooth to hurt when biting after a new filling?

Mild tenderness for a few days can be normal, especially after a deep filling.
