Root canal treatment: what it is and why it saves the tooth
What root canal treatment actually involves, why it relieves rather than causes pain, how long a treated tooth lasts, and when retreatment or extraction is the better option.

Root canal treatment has a reputation problem. It is remembered as the painful procedure when in fact it is the procedure that stops the pain.
What is actually happening
Inside every tooth is a pulp chamber containing nerve and blood vessels. When decay, a deep crack or trauma allows bacteria into that space, the pulp becomes inflamed and then infected.
The pulp sits inside rigid walls, so as it swells, pressure builds with nowhere to go. That pressure on the nerve is the severe, throbbing, keeps-you-awake toothache people describe.
Once the pulp dies, infection spreads through the root tip into the surrounding bone, forming an abscess. Left untreated this causes bone loss and facial swelling, and in rare cases becomes a medical emergency.
Root canal treatment removes the infected tissue, disinfects the canal system and seals it — eliminating the source.
The procedure, step by step
Diagnosis. Testing the tooth’s response and taking radiographs. A 3D CBCT scan is sometimes needed to identify unusual canal anatomy.
Anaesthesia and isolation. The tooth is numbed and isolated with a rubber dam, which keeps saliva and its bacteria out of the canal — the step that most affects success.
Access and cleaning. A small opening is made and the infected pulp removed. The canals are shaped with fine instruments and irrigated with disinfecting solutions. This is where the real work happens: canals are narrow, curved and often branched.
Sealing. The disinfected canals are filled with a biocompatible material to prevent re-entry of bacteria.
Restoration. The tooth is rebuilt, usually with a crown to protect it against fracture.
Many cases complete in a single visit. Severely infected teeth sometimes need two, with medication placed between.
Why the crown is not optional
This is the point most often misunderstood. A root-treated tooth no longer has a blood supply and has lost structure from both the decay and the access cavity. It becomes measurably more brittle.
A crown holds the remaining walls together and distributes force. Teeth that are root-treated and left without proper coverage frequently split — and a vertical root fracture usually cannot be saved.
When extraction is the better answer
We would rather say this honestly:
- The tooth has too little remaining structure to restore predictably
- There is a vertical root fracture
- Severe bone loss from gum disease has already compromised support
- Retreatment has failed and surgery is not viable
In those cases a dental implant usually gives a better long-term outcome than heroically retaining a failing tooth.
Aftercare
Mild tenderness when biting for a few days is normal and settles. Avoid chewing hard food on the tooth until the final restoration is fitted. Contact us if pain increases after three days, swelling develops, or the temporary filling comes out.
The signs that need attention
- Prolonged sensitivity to hot or cold that lingers after the stimulus is removed
- Pain on biting or when releasing a bite
- A tooth that has darkened
- A recurring bump or spot on the gum
- Swelling in the face or jaw — see someone urgently
Catching pulp inflammation early sometimes allows the pulp to be preserved. Waiting rarely improves the options.

Is a root canal painful?
The procedure is performed under local anaesthetic and is not painful. The severe pain people associate with root canals is the pain of the infected pulp *before* treatment — the treatment is what relieves it. Mild tenderness for a few days afterwards is normal.
How long does a root-treated tooth last?
A well-treated tooth restored with a crown can last decades. The main causes of later failure are fracture of the remaining tooth structure and re-infection through a leaking restoration, which is why the final restoration matters as much as the root treatment itself.
Why do I need a crown afterwards?
A root-treated tooth has lost its blood supply and often significant structure, which makes it more brittle. A crown distributes biting force over the whole tooth instead of concentrating it on weakened walls. Skipping the crown is the most common reason these teeth fracture.
Can the infection come back?
Yes, though it is uncommon when treatment is thorough. Recurrence usually means a canal was missed, the seal has leaked, or a crack has developed. Retreatment is often possible and is assessed with a 3D scan.
